THE ROLE:
In this role, you will provide meaningful and necessary coordination for all multi-faceted services for Regional Center consumers. You will collaborate with internal and external partners to build strong relationships and enhance program impact. Your day-to-day responsibilities will include:
- Develop and execute personalized care plans: Schedule intakes for new consumers, create individualized service plans based on their care needs, and update plans in response to changes in need
- Manage service requests and provider scheduling: Process incoming client requests for services, select and schedule providers for consumers based on scheduling and health needs, and screen potential direct support providers
- Facilitate training for direct support providers: Conduct ongoing training, including Crisis Prevention Intervention Training, to ensure direct support providers are equipped to deliver quality services
- Maintain compliance and confidentiality: Ensure compliance with all relevant laws and regulations, including HIPAA, and secure all protected health information
- Foster positive relationships and communication: Attend staff meetings and planning team meetings, maintain a positive outlook, and foster open communication with consumers, providers, and team members

WHAT YOU BRING TO THE TABLE:
- Spanish fluency is required
- Excellent written and verbal communication skills to help develop/maintain relationships and present clear and concise ideas
- Strong problem-solving abilities with a solution-driven mindset
- Exceptional organizational skills with the ability to balance competiting priorities while maintaining precision and accuracy when overseeing/reviewing critical information or documents
- Experience with Microsoft office is preferred
- Detail-oriented with strong analytical skills to identify problems and offer solutions for moving forward in projects
